a) 1" x 1" photo of player for Colorado Youth Soccer Player Identification Card 
b) Copy of player’s birth certificate or passport
c) Colorado Youth Soccer form
d) Signed Medical Release Form
e) Signed Fee Disclosure Form

     U16-U18 Tier II
f) Signed Player Code of Conduct
g) Payment with a check or credit card. The BCF offers a payment plan options and a financial assistance application.
